Outreach Officer/Case Worker – National Council On Drug Abuse

JOB PURPOSE

The Outreach Officer/Case Worker will have primary responsibility to reach, test, coordinate medical and social services, thus improving the clients’ quality of life and maintain continuity of care by other partners.

 

RESPONSIBILITIES (TECHNICAL/ADMINISTRATIVE/PROFESSIONAL)

The officer will be required to:

  • Interview and assess positive persons referred and obtain general social, medical and substance history and information
  • Develop a detailed plan of action for the Clients identified for intensive care, medical referrals, social interventions, skills training, and ensure proper follow ups and referrals
  • Liaise and collaborate with relevant service providers to offer a comprehensive package of care to the identified Clients
  • Create and maintain detailed confidential records of each client to include updates on referrals, medical visits, rehabilitation, calls, site visits, family visits, etc.
  • Organize and conduct programmes for the target population and project stakeholders
  • Coordinate data collection and management of the target population.
  • Participate in training sessions as organized by the Agency.
  • Conduct pre and post-test counselling for HIV testing.
  • Complete appropriate weekly and monthly reports.
  • Complete any other duties assigned

  • Confidentiality of information obtained on the job is preserved
  • Key outputs/deliverables are produced within agreed time frames to the required standards.
  • Individual Workplan is prepared within the established format and time frame and supports the strategic objectives of the component.

 

REQUIRED COMPETENCIES

  • Highly organized;
  • Ability to draft well-written correspondences;
  • Analytical thinker with strong written and verbal communication skills;
  • Ability to work under tight deadlines.
  • Proficiency in the use of relevant computer applications.
  • Excellent people management skills
  • Excellent Information Analysis skills

 

MINIMUM REQUIRED EDUCATION AND EXPERIENCE

  • A Degree in Social Work or related field
  • Training in Substance Abuse Treatment and Prevention
  • Certificate in Counselling
  • Training in HIV/AIDS
  • Minimum of three (3) years working in the field of behaviour change

 

REPORTING RELATIONSHIP

  • The Officer will report to the Project Coordinator (NCDA) or his/her designate and will work closely with Project representatives from the Ministry of Health and Wellness and other partners.

 

SPECIAL CONDITIONS ASSOCIATED WITH THE JOB

  • Maybe required to work beyond normal working hours in order to meet deadlines.
  • Will be required to travel island wide to execute job functions
  • Exposure to highly confidential and sensitive information
